Source: Centers for Medicare & Medicaid Services (CMS), Provider Information,
Ownership and Skilled Nursing Facility Cost Report datasets and the HCRIS
skilled nursing facility cost report files. Each raw manifest records the
exact URLs, edition, retrieval time and SHA-256 hashes. The official source
data dictionaries are retained internally by the publisher.

CMS states that the public reporting data are works of the U.S. government,
in the public domain, and may be reused without permission. Credit CMS.
Do not suggest that CMS or HHS endorses this derived dataset or its users.

The data.cms.gov catalog lists U.S. government works as the license for the
Skilled Nursing Facility Cost Report files: https://www.usa.gov/government-works
(currently published at https://www.usa.gov/government-copyright). That page
also notes that government material must not be used to imply endorsement and
that other people may hold publicity or privacy rights.

CMS publishes the Healthcare Cost Report Information System (HCRIS) files on
its cost reports page without a separate license; as federal agency
publications they follow the U.S. government works rules above. The page
carries this disclaimer: "Derived conclusions and analysis generated from this
data are not to be considered attributable to CMS or HCRIS." It also states
that CMS is not responsible for data that is misrepresented, misinterpreted or
altered. The captured page, the public use file methodology and data
dictionary, and the HCRIS documentation archive are kept with each raw
snapshot.

This collection contains published facility, ownership and cost report
records. It does not contain patient records, government logos or third-party
imagery. Public availability of a contact does not establish consent to
marketing. No exclusive rights in the underlying government facts are asserted.

Modifications: normalized column names, whitespace, typed dates, numeric and
boolean representations, telephone formatting, exact-CCN joins and chain
aggregates. Cost report values keep their published amounts; we added ISO
dates, HCRIS report status and dates linked by report record number or exact
CCN and period, Medicare and Medicaid day shares computed from published days,
exact-CCN facility links and a most recent report per facility. These derived
fields are our analysis, not statements by CMS. Original source bytes and
original source-row ordinals remain available for audit.

OVERTURE PLACE ENRICHMENT

Place fields come from Overture Maps Places. The Overture Maps Places notice
that follows gives the source licenses and attribution.

Modification notice: We selected US/territory place records and enrichment
candidates, linked them to records in this dataset using the labeled exact-key
rules named in place_match_method (name, phone, street and ZIP), serialized selected fields into
CSV and JSON text, formatted phone numbers, websites and email domains, and
withheld published email addresses other than shared business mailboxes. Match
fields are our derived evidence, not statements by the original sources.

Place data describe the Overture release, not a business or facility at an
earlier date. Coordinates, where included, are bounding-box minima rather
