FEATURES AND PREPARATION
- Every record in the OSHA ITA Form 300A summary file for one complete calendar year, as published with submissions through December 31 of the following year; no record is dropped or merged.
- Establishment and company names, street address, city, state and a five-digit ZIP, with the published ZIP kept beside it and leading zeros restored only where the state's ZIP prefix makes them certain.
- EIN as nine-digit text when the published value passes the IRS prefix syntax check, with the published value and a status for every record, ready to join employer datasets by EIN.
- NAICS codes decoded to Census 2022, 2017 or 2012 titles and sectors, preferring the edition the establishment reported, with the match type stated.
- Size class, annual average employees, total hours worked, deaths, days-away, job transfer or restriction and other recordable cases, days counts and injury and illness types from Form 300A.
- OSHA Total Case and DART rates per 200,000 hours computed with OSHA's published formula, plus flags for internally inconsistent employee, hours and case values.
- Duplicate groups by normalized name, street and ZIP, with identical-record counts and a latest-submission flag, so repeated submissions can be reviewed without losing records.
- Current place links by labeled exact-key rules (establishment name, core name, street, building and ZIP), with websites, phones, shared-mailbox emails, place categories, brands, social profiles and per-place attribution; ambiguous candidates stay unlinked.
- Exact-EIN links to the employer's Form 5500 plan sponsor record (business phone, plan counts, participants, 401(k)/health flags, plan assets, carriers, recordkeepers) and to IRS exempt-organization records (subsection, NTEE, latest Form 990 website, phone, employees).

LIMITATIONS
- The CY period label denotes the calendar year in which injuries and illnesses occurred; OSHA collected the summaries through December 31 of the following year, and later submissions are absent.
- Only establishments required to submit Form 300A electronically, or choosing to, are included: generally 250+ employees in recordkeeping industries and 20-249 employees in designated industries. The data are not representative of all establishments or industries.
- OSHA does not validate employee, hours or case counts. Rates and flags repeat the published values; OSHA advises against labeling establishments most or least dangerous from these rates alone.
- Recording or reporting an injury, illness or fatality does not mean that the employer or employee was at fault, that an OSHA rule was violated, or that the employee is eligible for benefits.
- EINs are syntax-checked only; an eight-digit EIN may have lost a leading zero at the source and is not restored. Names and addresses are submitter entries, not verified legal identities.
- The publisher exported ZIP codes as numbers; leading zeros are restored only for states whose ZIP codes all begin with zero, and other short values stay blank beside the published text.
- Duplicate groups can hold repeated submissions or distinct establishments sharing a generic name and address; records are never removed. The ITA establishment ID can change when a site creates a new profile.
- Case detail (Forms 300 and 301) data, OSHA inspections, citations and penalties are not included.
- Place contacts describe the Overture release, not the injury year. Matched does not mean manually verified; emails are limited to published shared business mailboxes, and establishments with generic names such as Main Office often remain unmatched.
- Linked Form 5500 and IRS values describe the whole employer EIN, not the establishment; they are exact-EIN joins from the captured sibling editions, not verified identity.
